Commitment to Patient-Centered Care

"My role as a primary care provider is to support and guide patients to their highest capacity for health and wellness. I find that this is best achieved through a team approach in which barriers to wellness are reduced or eliminated. It is my firm belief that the patient is an integral member of the health care team whose priorities and values must be honored."

Clinical Expertise

Koleena has been in the nursing field since 2004, with the majority of that time spent working with underserved urban populations in various roles. In 2013, she completed her master's degree in nursing at Winona State University and an internal medicine internship at Manipal Hospital in Pokhara, Nepal. After returning to Minnesota, she began working as a nurse practitioner. She enjoys working in an integrated, multidisciplinary team to address complex psychosocial needs that may affect a patient's access to and engagement in primary care.
