Multi-Service Outpatient Program
This service is for patients who continue to need and benefit from two or more therapies, but do not require these therapies more than once a day. These outpatient therapy services are usually scheduled two to three times a week either in the morning or the afternoon, but can also be daily. Social services are available to assist our patients with coordinating their medical care as well as other issues that occur during rehabilitation. Services may include physical therapy, occupational therapy, speech language pathology, clinical psychology and/or neuropsychology.
Single-Service Outpatient Program
Single service is for outpatients who still require and benefit from one specific therapy. The patient is only seen by one therapy. The frequency of attending the therapy can range from once a week to several times a week. Social services are available to assist our patients with coordinating their medical care as well as other issues that occur during rehabilitation. Services may include physical therapy, occupational therapy, speech language pathology, clinical psychology and/or neuropsychology.
Discharge planning starts the day a patient is admitted to the Miland E. Knapp Rehabilitation Center. When treatment and nursing goals have been achieved or continue treatment as an outpatient is appropriate, a safe discharge plan will be discussed with the patient and family.
Most discharges occur shortly after a successful overnight pass or after the final family conference.
The patient will receive a discharge packet with medication and care instructions as well as follow up appointments for primary care and/or specialty clinics. Arrangements for public health nurses and home-based therapy can be made as approved by insurance providers. Patients who live in close proximity to HCMC often choose to continue their outpatient rehabilitation program at the Miland E. Knapp Rehabilitation Center.
Patients often have therapy needs after being discharged from inpatient rehabilitation. We offer two treatment program options for patients to consider depending on those needs, including:
Multi-Service Outpatient: Physician orders for two or more therapies, specializing in Brain Injury and Stroke.
Single-ServiceOutpatient: Physician orders for single service or one therapy.
Most patients require some outpatient therapy after discharge either at home or other facilities closer to home. Therapies may include:
- Physical Therapy
- Occupational Therapy
- Speech-Language Pathology
- Therapeutic Recreation
If a patient does not live close to HCMC, a staff member will help locate an appropriate outpatient rehabilitation program in or near his or her home community.
If home discharge is not possible, the team will make recommendations for the next step to meet the patient’s continued rehabilitation needs.
Day and Overnight Passes
A home pass is highly recommended close to discharge. This will allow the family and rehabilitation team the chance to see how the patient functions in the home and community. A pass may also help with discharge planning. Sometimes unforeseen problems occur during a pass. A pass questionaire will be given to patients and families with questions pertinent to how that pass went. This gives the family, patient, therapists and physician an opportunity to correct any problems encountered on the pass prior to discharge home. Community and home passes also help motivate the patient for the transition home and for increased independence.
At three to four months after discharge, patients will receive a confidential over-the-phone survey. This survey is important for several reasons. We want to determine if patients are satisfied with the Knapp Rehabilitation Center. We also want to determine how the patient is functioning after discharge. At the time the survey is given the majority of patients are improving or stabilizing. The follow-up survey information is meaningful to the Knapp Rehabilitation Center so we may continually improve our services for each patient.
