Community Involvement

Hennepin County Medical Center is recognized for providing healthcare services to patients in our neighborhood clinics and medical center in downtown Minneapolis. But our connections to the community take us well beyond the walls of the hospital or clinics.

We are committed to understanding the needs of the communities we serve, going into the community to meet those needs, and engaging in a conversation with the community along the way.

Community Health Needs Assessment

As part of the Affordable Care Act, all non-profit hospitals conduct a Community Health Needs Assessment (CHNA) every three years to identify current and emerging top priority health needs in the communities they serve.  HCMC’s most recent CHNA was conducted in 2016 and involved analysis of existing MN and Hennepin County data as well as input from a broad range of community stakeholders. Top identified priority health needs were: mental health, social determinants of health, and maternal child health.

At the completion of the CHNA, hospitals create a 3-year implementation plan to address the identified needs. The 2017 – 2019 implementation plan will be available in May 2017.

Health Services Plan

The Health Services Plan is a unique requirement to Hennepin Healthcare System. The 2017 plan is aligned around the three priority health needs identified in the 2016 CHNA: mental health, social determinants of health, and maternal child health.   The plan features new or expanded efforts to impact HCMC patients and families.

We have a sustainability reporting process to affirm our mission of providing access to quality care for all, improved environment for our staff, and stewardship of the cost of healthcare.
